The big boot keeps out a lot of heat while sealing a big open area on the transmission tunnel. The inner boot (also known as the turret seal) seals the shifter onto the transmission. It keeps gear oil from coming out and destroying the big boot as well as gear oil odor. 
 
Changing both at the same time is recommended but not necessary, a torn inner boot is known to cause the big boot to deteriorate.
